Creating Opportunities is honored to be partnered with Yamhill Community Care (YCCO) to connect YCCO Members with Health-Related Social Needs (HRSN) Benefits which include a variety of services to enhance their wellbeing.
HRSN Benefits
With Health-Related Social Needs Benefits, you may be eligible to receive help with the following:
- Outreach & Engagement – Assistance accessing benefits such as TANF, SNAP, WIC, and other basic needs, as well as connecting with services for healthcare, peer support, social engagement, education, legal and more
- Nutrition – Healthy meal delivery or nutrition counseling
- Housing – Rent assistance and utility assistance
- Home Changes for Safety – medically-necessary accessibility modifications (ramps/grab bars, pest control, health-related heavy-duty cleaning, etc.)
- Home Changes for Health – Air Conditioners, heaters, air filters, mini fridges for medication storage, and portable power for medical devices.
Eligibility
To qualify for HRSN services, individuals must meet all the following criteria:
- Be a current Oregon Health Plan (OHP) member.
- Be presumed to be part of an HRSN Covered Population.
- Be presumed to have an HRSN Clinical Risk Factor.
- Have a corresponding Social Risk Factor linked to the requested benefit.
- Be a Yamhill CCO member.
